Policies
Appointment instructions A link will be sent to you for the patient portal. Once you receive the link, please open it and complete the new patient intake forms. These forms must be completed before the first appointment. You must upload you ID and insurance card front and back . You can do this by sending a private message via the portal. The portal is also a great tool for reaching the office team. Thank you . We look forward to working with you.

Identification Policy
All patients must present a valid, non-expired state issued Driver's License, state issued ID card, US Passport or Passport ID card.
Cancellation Policy
We know your life is busy and we value your time. With that in mind, our office does not double-book patients and requires 24-business hours notice to cancel or change an appointment. If proper notice is not provided, a no show fee of $75 will be charged.
Confidentiality Policy
Ultimate Psychiatry takes patient privacy very seriously. As part of our new patient packet, the full HIPAA-Patient Privacy Notice will be provided, and each patient will acknowledge and sign off of their review of the policy.

Prescription Policy
Our office provides refills and renews prescriptions only during appointments. This practice reduces prescription errors, improves patient safety and encourages appropriate follow-up. Patients will receive enough medication or refills to last until their next recommended follow-up. Patients are asked to track their supply, and to ensure they have an appointment scheduled before they run out of medication or run out of existing refills. Please be proactive in your care and track how much medication you have and how many refills remain on the prescription, and ensure you have an appointment to see the doctor before you're out of medication.
Lab Work Request Policy
To ensure patient safety and quality, lab work must be completed within 24 hours from your office visit and/or as requested. If available, our office will work with your Primary Care Provider (PCP) to obtain your current lab work. This will aid in our coordination of care efforts and reduce duplicative lab orders.

Insurance
We accept most insurance plans including Medicare, Medicaid, BlueCross Blue shield, Aetna, Cigna, United Healthcare plans, to name a few. Our staff will help you with insurance questions and scheduling.
Rates Without Insurance
For patients without insurance, we offer a prompt pay discount. Medical expenses are tax deductible. Our cash price is $300 for a new evaluation and $150 for follow ups.
Payment Options
Cash, check, Visa, Mastercard, American Express, Discover, Cash App, PayPal, Zelle, and Apple Pay are accepted for payment. Health Savings Accounts (HSA) are also accepted.
